Even though Americans are living longer, healthier lives than ever before, many will experience a serious chronic illness or disability in the later stages of life. It’s important that people plan ahead for the health care that they want, so in 2004, we launched the Sharing Your Wishes program.
Through the Sharing Your Wishes program, we identified and supported a total of 13 community coalitions that worked to make older adults aware of the importance of planning in advance for their health care in the event that they experience an illness or condition that prevents them from making or communicating decisions.
This summary report provides background on our approach and the design of Sharing Your Wishes and includes coalition case studies as well as lessons learned from the three-year program.
