Sustaining Sharing Your Wishes

In 2004, we launched the Sharing Your Wishes program to identify and support community coalitions in facilitating improved health care decision-making for older adults. We initially funded six coalitions, and as the work developed, we expanded the program to seven other counties.

We worked with the coalitions to identify a number of existing best practices, including patient-centered care, palliative care, coordinated approaches to care and community and family involvement in improvements to quality of care and quality of life. We also developed a comprehensive set of Sharing Your Wishes materials for the coalitions to use and distribute.

In an effort to assess the ability of the coalitions to sustain their successful efforts, we allocated resources to support tracking and reporting for three years after the project funding ended. This report documents the program activities that coalitions continued and describes the methods they used to promote them over a three-year period from 2008-2011.
