As part of our 2007-2008 Collaborative to improve transitions of care, 13 teams implemented the Care Transitions Intervention developed by Dr. Eric Coleman of the University of Colorado Health Sciences Center.

The Collaborative’s goal was to stimulate change in practice and care delivery systems to improve transitions for older adults as they moved from one care setting to another. Better management of these transitions improved continuity, reduced error and delay, and increased a person’s control over his or her health decisions.

This evaluation summary includes highlights from each team’s work during this Collaborative.
