Who We Are
Founded in 2002, the Health Foundation for Western & Central New York awarded its first grants in April 2004. Since that time, more than 400 grants totaling more than $30 million have been awarded to fund programs in 16 counties in western and central New York.
Our mission is to improve the health and health care of the people and communities in western and central New York. Our vision is a healthy central and western New York where racial and socioeconomic equity are prioritized so all people can reach their full potential and achieve equitable health outcomes.
To achieve outcomes that last, we work together with our community partners to develop, implement and evaluate sustainable programs that make a positive difference in the lives of thousands of young children and older adults in our regions, and support the community-based organizations that serve them.

The Foundation is seeking an entry level full-time Program Manager to assist in identifying potential Foundation partners, assess incoming grant requests, develop recommendations, and present promising proposals for funding approval across its 16-county region. This position will also provide monitoring and oversite of funded grants across the 16 counties and assist in the management of Foundation programs, initiatives and projects.
The successful candidate will be comfortable working with diverse populations in urban, suburban and rural settings, exploring new ideas and concepts, have an innovative spirit, a commitment to and passion for the Foundation’s mission, an affinity for collaboration and teamwork, as well as a broad knowledge of the healthcare sector. This position requires a self-starter and strategic thinker.
A Bachelor’s degree and a minimum of four years working in healthcare, and/or nonprofit community-based health and human services preferably in a program development capacity; strong organizational skills; excellent communication skills (verbal and written); and the ability to collect and analyze information, problem-solve, and make decisions. This position requires proficiency in Power Point, Word, Excel and Outlook.
The Program Manager will be housed in the Foundation’s Buffalo office with a starting salary of $60,000 – $65,000 depending on education and experience. The Foundation also offers a competitive benefits package. The anticipated start date is May/June 2021.
Due to the nature of the position, some travel is required.
