The Health Foundation for Western and Central New York awarded new grants totaling more than $3.6 million for the last half of 2014, Foundation President Ann F. Monroe announced Tuesday, February 3, 2015.
The Foundation’s grant making activities reflects its commitment to three focus areas: improving the health and health care of young children impacted by poverty and vulnerable older adults; and supporting the systems that serve them.
The following grants were awarded:
Children Impacted by Poverty: All children impacted by poverty are physically, socially and emotionally healthy as they enter kindergarten.
Niagara University, Niagara Falls, NY – $299,035 to support Help Me Grow Western New York, an initiative that connects at-risk young children with services they need.
County of Chautauqua, Mayville, NY – $10,000 for the Chautauqua County Department of Health & Human Services to increase community supports for breastfeeding within hospitals, primary care practices, workplaces and at home.
The following grantees were awarded up to $80,000 as part of the Health Foundation’s Maternal and Child Health Expanding Best Practice Initiative in western New York.

Research Foundation of SUNY, Albany, NY – Up to $80,000 to expand the current successful Centering Pregnancy prenatal care program to include the Midwifery Center on the Community Campus
Reach CNY, Syracuse, NY – Up to $100,000 to implement SBIRT screening to women of childbearing age
The Salvation Army, Syracuse, NY – Up to $100,000 to evaluate and expand implementation of the Strengthening Families Program.
Upstate Medical University, Syracuse, NY – Up to $100,000 towards its Centering Pregnancy for refugee/migrant women at the Midwifery Center.
The following grantees were awarded $20,000 each as part of the Health Foundation’s children’s oral health efforts. These grantees will become a new Cavity Free Kids hub and will train and support early child care programs in successfully using Cavity Free Kids curriculum to help young children impacted by poverty develop lifelong healthy dental habits and better oral health.

The following grantees were each awarded grants of up to $100,000 as part of the CHOMPERS! Portable Dental Care Expansion, which brings dental treatment to children under the age of five to locations where they already congregate.

- DentaQuest Institute Safety Net Solutions, Westborough, MA – Up to $54,180 to provide support, education and technical assistance for up to six grantees interested in developing a portable dental care program.
Vulnerable Older Adults: All vulnerable older adults are able to plan for and maintain a dignified, independent, high-quality life in their community.
New York Academy of Medicine, New York, NY – $50,000 to evaluate Project ECHO, a program designed to increase rural primary care providers’ capacity to treat depression in older adult patients at the University of Rochester Medical Center.
Healthy Community Alliance, Inc., Gowanda, NY – $94,000 to initiate a collaborative planning effort to engage groups and organizations with an interest in creating a sustainable, regional approach to providing senior services in the southern Erie County and northern Cattaraugus County.
Healthy Community Alliance, Inc., Gowanda, NY – $15,000 to coordinate a Medication Therapy Management Readiness Plan.
United Way of Buffalo and Erie County, Buffalo, NY – $45,000 to support the Erie & Niagara Aging Network Steering Committee in exploring the feasibility of establishing a more formal network structure to contract with Managed Care Organizations.
The Community Foundation of Herkimer & Oneida Counties, Inc., Utica, NY – $50,000 to use a concept mapping process to gain input from older adults about what would help them remain longer in their neighborhood.
Community Health Capacity: All communities are able to effectively plan for, and address, health needs of the most vulnerable and those in poverty.
Central New York Community Foundation, Syracuse, NY – $142,000 for the development, implementation and management of a Syracuse Green and Health Homes Fund for the City of Syracuse.
Healthy Community Alliance, Inc., Gowanda, NY – $172,586 to support the continued development of the Fellows Action Network (FAN).
Family Medicine Medical Service Group, LLC, Syracuse, NY – $14,900 to explore different models of delivering primary care for the center.
Center for Family Life and Recovery, Inc., Utica, NY – $10,000 to hire a consultant to guide implementation of the organization’s strategic plan.
Northern Oswego County Health Services, Inc., Pulaski, NY – $70,000 to engage a consultant to coordinate the collective Design System Reform Incentive Payment (DSRIP) activities among the participating central New York Federally Qualified Health Centers.
Clean Air Coalition of Western New York, Buffalo, NY –$1,500 to support strategic planning and implementation for the organization.
The Chautauqua Center, Dunkirk, NY– $18,000 to hire a consultant to develop the New Access Point (NAP) application.
Rochester Primary Care Network, Rochester, NY – $52,000 to support an evaluation of current and future workforce needs for the Health Center and develop a business plan.
Children First Scholarship, Buffalo, NY – $400,000 in scholarships to help families with young children in low performing Buffalo Public Schools transfer to a non-public school of their choice.
University of Colorado Denver, Denver, CO – $100,000 to evaluate the impact of the Get Well/Stay Well model implemented by Niagara Falls Memorial Medical Center.
P2 Collaborative of Western New York, Inc., West Seneca, NY – $10,000 to provide technical assistance to develop an application to the New York State Department of Health for a Population Improvement Planning contract for the eight counties of western New York.
Center for Family Life and Recovery, Inc., Utica, NY – $40,000 to support implementation of an updated strategic plan that aligns with the agency’s needs for managed care readiness.
Evergreen Foundation of Western New York, Inc., Buffalo, NY – $150,000 to provide clinical and organization support to SNAPCAP (Safety Net Association of Primary Care Providers) to ensure progress on critical projects.
Allegany/Western Steuben Rural Health Network, Inc., Wellsville, NY – $15,000 to build upon its already existing medication management project established as part of the local Step Up to Stop Falls initiative.
Center for Hospice & Palliative Care, Buffalo, NY – $10,000 to hire a consultant to explore and facilitate potential forms of collaboration that would serve both Hospice Buffalo and Hospice Chautauqua County’s respective missions.
The Gifford Foundation, Syracuse, NY – $15,000 to support a workshop focused on training central New York not-for-profits in the importance, art and skills of storytelling.
WRVO Public Media, Oswego, NY – $54,376 to sponsor the Take Care Radio Series, a half-hour health series developed and aired by WRVO Public Media weekly on Sundays.
The following grants, totaling $580,000, were awarded as part of the Health Foundation’s Ready or Not Cohort 2 initiative in central New York. The following organizations will be assigned an organizational development consultant who will work with them for up to two years to develop and implement a capacity building plan:
