To celebrate our 10-year anniversary, we took a look back at where we have been as a Foundation, what we have accomplished together with the people and communities we serve and look forward to what is on the horizon.
Throughout the publication, we focus on the growth and development of the Health Foundation’s three focus areas: Vulnerable Older Adults, Children Impacted by Poverty and Community Health Capacity and highlight several of our key programs from each.
We also introduce you to some of the individuals and organizations that are making a difference in the lives of those that are most vulnerable. You can hear more about their experiences in a video featured here.
