Released in 2007, when we were known as the Community Health Foundation of Western and Central New York, our first Report to the Community presents a look back across the foundation’s first five years and reflects on what progress we made and what we learned along the way.
Focused on four themes, Living, Leading, Learning and Lasting, the Report to the Community provides information about our foundation’s history, focus areas and vision for improving the health and health care for the people and communities of western and central New York. We also give an overview of some of our key early programs that laid the groundwork for much of the work we are doing as a foundation today.
